Complete the Physician Workforce Survey

1. Login to online services by selecting your profession from the dropdown menu and entering your User ID and Password Your User ID and Password were mailed with your initial license. Please look at the center section and refer to the Online Services Instructions, item #5. If you do not have your User ID and Password, click on “Get Login Help”. 2. Review the Status of My Application

You may check the status of your application in real time via our online services, using your User ID and password. If you lose your User ID and password, contact Licensure Support Services at (850) 488-0595 to get this information. Note: We are in the process of making this service available to all professions. However, if your profession is not yet online, you must contact the board or council office directly to check on the status of your application.

Request Certified Documents

If you need a written statement on a public record attesting to the record’s genuineness or that it is a true and correct copy, you may fill out the online request form. Be sure to indicate you need a certified copy of the request form. A $25 fee will be charged, in addition to the public record fees and charges. Visit our Public Records page for information on how to request certified documents by mail.

Print a Confirmation of License

You can print a confirmation of license through the Practitioner Login feature of MQA Services. The confirmation is available up to 30 days after you submit your online renewal request. After logging into the system with your User ID and password, select Print Confirmation of License from the navigation bar located on the left. If you receive an error when trying to print your temporary license, the file was most likely corrupted during the download process. Please view these instructions on how to fix this problem. Scroll down until you find the internet browser you are using.

Processing Time

If you are submitting a name or address change request by mail, please allow 5-7 business days for processing. Online requests will be processed with in 48 hours. Please Note: An updated license will not be automatically sent. Beginning July 1, 2023, licensees will be able to retrieve duplicate licenses, at no cost, by logging into the MQA Online Services Portal. The PDF version of the license will be available under the “Licensure Documents” menu on the licensee’s personal dashboard. Should you need a duplicate license prior to July 1st, please reach out to the MQA Customer Contact Center at 850-488-0595 for assistance. If you need to change your name, and you prefer to renew online, please submit your name change request by mail and allow 5-7 business days processing time before you renew online.

Update My Address

1. Login to online services by selecting your profession from the dropdown menu and entering your User ID and Password Your User ID and Password were mailed with your initial license. Please look at the center section and refer to the Online Services Instructions, item #5. If you do not have your User ID and Password, click on “Get Login Help”. 2. Select “Update Addresses” on the left side of the page 3. Enter the new address information 4. Once you have entered your new address, click on “Process”. You will receive a confirmation page that displays the updated address. If you prefer to change your address by mail, please complete the Change of Address form. Written requests for address changes must include your name, profession, license number, old address, new address, date of birth, last four digits of your social security number, and your signature. About Your Practice Location Address The practice location address will display on the Internet and your license. Your practice location must be a physical location address and must not include a Post Office box. The mailing address will only display on the Internet if you have not provided a practice location address to us. Establishment/Facility Name or Address If the name or address change is for a facility that has changed location, a licensure application must be submitted. The testing vendor will not allow me to sit for the exam because the name they have for me does not match the one on my ID. What can I do about this?

Submit a written request to the board office to have your name changed, along with a copy of your driver’s license or other official documentation showing your correct name. Staff will make the needed changes and provide the corrected information to the testing vendor.

I have already been authorized to sit for the exam(s), but when I tried to schedule, the testing vendor would not let me. They said I’m not on the list. What happened and what can I do?

Depending on when you were authorized to sit, your authorization may have expired. Your authorization will have to be resubmitted. Please notify psychology board staff and request your authorization be resubmitted.

Am I allowed to review my Florida laws and rules exam to see which Questions I got wrong?

Where is the Florida laws and rules exam administered?

Please visit Pearson VUE’s website at http://www.pearsonvue.com/fl/doh/ and use “Find a Test Center” map feature to identify the testing site(s) nearest you.

How many times per year are the exams administered?

The exams are administered on a continual basis. You may select the date that is convenient for you.

Is there any application method that will exempt me from taking the Florida laws and rules exam?

No, all methods require it.
